Key Responsibilities\n

• Conduct thorough interviews and assessments of patients and their families to identify key social, emotional, and economic factors affecting their healthcare experience.

• Utilize evidence-based, trauma-informed therapeutic techniques to provide telehealth mental health services tailored to individual client needs.

• Advocate for and connect clients to vital community resources, including food assistance, healthcare services, and mental health support.
\n
• Monitor client situations proactively, ensuring consistent follow-up and adjustment of care plans aimed at improving mental wellbeing.

• Handle crisis situations such as mental health emergencies or potential child abuse with sensitivity and professionalism, ensuring the safety and wellbeing of all parties involved.

• Assist clients in adapting to life changes associated with their or loved one's illness, financial stress, or other significant life transitions.

Patient Population / Client Focus\n

Your primary focus will be on clients within hospice care, where empathy and understanding are critical. You will work closely with individuals facing terminal illnesses and their families, helping them cope with grief, loss, and the overall emotional turmoil that accompanies end-of-life decisions. Experience and comfort in working with veterans will be especially valuable, as many clients may have unique histories that inform their needs.

Qualifications\n

• A Master’s degree in Social Work from a recognized institution.

• A valid and current LCSW license to practice in Pennsylvania.

• Minimum of 2 years of experience in mental health service provision, preferably in a healthcare-related setting.

• Familiarity with Department of Mental Health billing and paperwork systems.
\n
• Expertise in parent-child interventions and knowledge of child development principles.

• Excellent verbal communication skills to effectively counsel patients and navigate challenging conversations.

Compensation and Benefits\n
