RN Case Manager - Home Health

Provide high-quality home health care to patients in their preferred home setting Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating treatment plans Assess physical, psychological, social, and spiritual factors influencing patient, family, and caregiver health status and incorporate data into the plan of care Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit Make follow-up visits according to patient, family, and facility needs Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other care personnel Revise plans of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members Align all activities with Elara Caring’s vision and Home Health leadership Report to the Clinical Manager or Clinical Supervisor Requirements Current, unrestricted state RN license 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Dependable vehicle Valid driver’s license Current auto insurance in accordance with state laws Must be able and willing to travel 50% Associate’s Degree or Bachelor’s Degree in Nursing is preferred Previous Home Health or Hospice experience is preferred Previous experience with HomeCare HomeBase is preferred Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule Core Competencies Demonstrates expertise in patient assessment, care planning, and coordination in home health settings, with a strong focus on maintaining patient privacy and compliance with HIPAA regulations.
