Westgate Hills Rehabilitation and Nursing in beautiful Havertown, PA is hiring for a Part Time Social Services Assistant. This position is 20 hours per week. Job Description The Social Services Assistant collaborates with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services The Social Services Assistant participates in the development of a written, interdisciplinary plan of care for each patient/resident to address social services needs The Social Services Assistant hold meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life The Social Services Assistant performs patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers. The Social Services Assistant establishes relationships and maintains contact and referral flow with community based agencies/services
