We are looking for a compassionate and dependable **Social Services Assistant** to join our team and help residents and their families navigate their healthcare journey with dignity, compassion, and support.Why Join Us?* **$20.00–$25.00+ per hour, DOE*** **Full-Time*** Medical, dental & vision benefits* **401(k) retirement plan*** **Paid Time Off (PTO)*** Employee recognition & rewards* **Bonus opportunities*** Training and professional development* Career growth opportunities* Supportive and collaborative team environment* Convenient San Francisco location with **ample parking**What You'll DoAs a Social Services Assistant, you will play an important role in supporting our residents and families throughout their stay.Your responsibilities will include:* Build meaningful relationships with residents and families and provide compassionate support.* Assist residents with social, emotional, and psychosocial needs.* Support care planning and help identify goals that promote independence, dignity, and quality of life.* Provide emotional support to residents and families dealing with illness, disability, adjustment, grief, and loss.* Assist with **admissions, transfers, and discharges**.* Participate in discharge planning and connect residents with appropriate community resources.* Interview residents, guardians, and sponsors to gather required admission information.* Prepare admission packets, including resident rights, privacy notices, and admission agreements.* Maintain accurate resident records and identification information.* Maintain accurate bed availability records.* Work closely with nursing, rehabilitation, administration, and other interdisciplinary team members.* Maintain confidentiality and follow facility policies and applicable privacy requirements.What We're Looking ForWe're looking for someone who is **compassionate, organized, dependable, and genuinely enjoys helping people.****Requirements:*** High school diploma or equivalent required.* Previous healthcare, skilled nursing, senior living, or social services experience preferred.* **PointClickCare (PCC) experience is a plus, but not required.*** Excellent communication and interpersonal skills.* Strong organizational and documentation skills.* Ability to manage multiple priorities in a fast-paced environment.* Ability to work effectively with residents, families, staff, and community resources.* Empathetic, patient, professional, and resident-focused approach.Ready to Make a Difference?If you're looking for a rewarding career where you can **build relationships, support residents and families, and make a meaningful impact every day**, we'd love to meet you!**Apply today to join the Victorian Post Acute team.****Job Type:** Full-Time **Pay:** $20.00–$25.00+ per hour, DOE **Location:** 2121 Pine Street, San Francisco, CAWe are an equal opportunity employer.
