Key Responsibilities: As a Social Services Assistant , you’ll have the unique opportunity to make a real difference in the lives of our residents by: Assessing Needs: Complete assessments for each resident, identifying their social, emotional, and psychological needs. Collaborating on Care Plans: Actively participate in creating and updating the interdisciplinary care plan to ensure a comprehensive approach to each resident's well-being. Providing Support: help residents cope with the transition and adjustment to a long-term care environment, ensuring they feel supported throughout their stay. Family Support: Provide education to residents and their families, helping them navigate challenges and maintain strong connections. Discharge Planning: Coordinate the discharge process, ensuring appropriate home care services are in place for residents’ safe and successful transitions back into the community. What We’re Looking For: Educational Requirements: Associate’s or Bachelor’s degree in Social Work, Psychology, Sociology, or a related field preferred. Experience: Previous experience in social services or healthcare, especially working with elderly or disabled populations, is highly desirable.
