HomeWell Care Services of Middle Tennessee is looking for a Care Manager who loves helping people, stays cool under pressure, and wants to grow with a company that will grow right along with them. We’re a non-medical, in-home care company — which is a fancy way of saying we help older adults and people who need a hand stay safe, happy, and independent in their own homes. Our Care Managers are the glue between our clients, their families, and our caregivers, and it’s one of the most rewarding roles on the team. In this role you’ll coordinate and supervise the care our clients receive: matching the right caregivers to each client, making sure care plans are followed, checking in with families to keep everyone happy, and jumping in to solve problems before they become problems. - Schedule and match caregivers to clients based on needs, personality, and fit.
- Fill open caregiver shifts as needed to keep every client covered.
- Supervise client care to ensure caregivers follow the care plan and deliver safe, quality service.
- Check in regularly with clients and families to make sure they’re happy with their care.
- Make follow-up calls with new caregivers after first shifts — answer questions, catch issues early.
- Perform in-home client visits and reassessments as needed.
- Answer intake calls, schedule assessments, and re-schedule clients when a caregiver calls out.
- Watch for missed check-ins and make sure caregivers arrive on time.
- Keep client records current and accurate, and help train caregivers in the home, by phone, or in the office.
- Serve as a trusted resource for clients and families — education, referrals, and a friendly voice.
- Schedule: Friday through Tuesday (with Wednesday and Thursday off — hello, mid-week weekend!).
- Pay: $36,000/year salary.

- Some driving is required (you’ll need reliable transportation, a valid license, and current insurance).
- Some overnight on-call is part of the rotation.
- Some holiday coverage is required — caring for people doesn’t always take a day off, and we share the load.
- Two years of experience in home care, home health, or a related field.
- Impeccable follow-up and attention to detail — you track a lot without dropping the ball.
- Warm, clear communication with clients, families, caregivers, and coworkers.
- The ability to supervise, assess, and advocate for clients and families.
- A calm, solution-focused attitude and sound judgment when things get busy.
- A real passion for helping others live with dignity.
- Must pass a criminal background check.
