Work Environment

You will be primarily working on-site in the homes of patients within the Daytona Beach area, allowing you to provide personalized care in a comfortable and familiar environment. A standard workweek in this role entails a Monday to Friday schedule, usually summing up to 40 hours per week.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to determine their physical therapy needs.
• Develop and implement individualized treatment plans, focusing on orthopedic conditions, while utilizing evidence-based practices.
• Deliver direct care by applying various therapeutic techniques and modalities to patients in their homes, ensuring alignment with the physician's prescribed plan of care.
• Educate patients and their caregivers on treatment processes, self-management strategies, and preventative health measures.
• Regularly assess and document patient progress, adjusting treatment plans as necessary to help meet rehabilitation goals.
• Foster effective communication with a broader healthcare team, including physicians and other health professionals, to maximize patient outcomes.
• Uphold high standards of customer service, ensuring that each interaction with patients contributes to an exemplary healthcare experience.
• Meet minimum utilization goals and other performance-oriented business objectives.

Patient Population / Client Focus

The primary patient demographic includes individuals recovering from orthopedic surgeries, sports injuries, auto accidents, and various musculoskeletal disorders. In this role, you will handle a diverse range of patient needs, including those requiring specialized care in pediatric, neurologic, and sports medicine rehabilitation, allowing for a fulfilling and multifaceted work experience.

Qualifications

• Bachelor's degree in Physical Therapy from an APTA or CAPTE-accredited program.
• Current State License as a Physical Therapist in Florida.
• Basic Life Support (BLS) Certification from the American Heart Association.
• Proficiency in verbal and written communication as well as familiarity with basic computer and word processing skills.
• Ability to establish trusting rapport with patients, facilitating a therapeutic relationship that encourages their recovery.

Preferred Experience

While new graduates are welcome to apply, experience within a home care setting or in treating a range of orthopedic conditions will be viewed favorably. Experience in utilizing advanced modalities and rehabilitation technologies is a plus.
