Work Environment

This role is designed for a dedicated professional who will provide on-site, home care therapy. Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing medical histories and performing specialized assessments to determine individualized treatment needs.

• Communicate evaluation outcomes and treatment plans clearly to patients and their families, engaging them actively in the goal-setting process.

• Implement treatment protocols effectively, adapting strategies dynamically based on continuous assessment of patients' conditions within the evolving healthcare landscape.

• Collaborate with interdisciplinary teams, consulting when necessary about changes in patients' conditions that may require modification of care plans or additional medical consultations.

• Oversee discharge planning, ensuring a smooth transition back to everyday life for patients, and meticulously document patient care activities across all treatment phases.

• Exhibit excellent time management skills to meet productivity expectations while maintaining an organized schedule and punctual attendance.

• Contribute actively to the department's strategic goals and objectives, adhering to all governmental and accreditation standards.

• Engage in ongoing professional development, pursuing continuing education to enhance skills and maintain best practices.

• Deliver outstanding customer experiences by being flexible and responsive to patient needs, strengthening access to care initiatives.

Patient Population / Client Focus

Your primary responsibility will be working with a diverse range of patients in home care settings, including post-surgical rehabilitation, orthopedic injuries, and chronic pain management. Each patient's unique circumstances and preferences will guide your treatment approaches, ensuring that care reflects a deep understanding of their personal goals and recovery journeys.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.

• Required licensure to practice as a Physical Therapist in the state of Florida.

• Current certification in Basic Life Support (BLS) for Healthcare Providers as per American Heart Association standards.

• While prior experience is not mandatory, a passion for patient care and a willingness to learn in a dynamic environment is essential.

Preferred Experience

• Experience in outpatient orthopedic therapy or home health services is a plus but not required.

• Familiarity with electronic health record systems and documentation practices is advantageous.

Compensation and Benefits
