Overview
Licensed Practical Nurse (LPN) – Long Term Care in Clifton Springs, NY. This travel assignment starts on 08/18/2026 for 13 weeks, with 5x8 evening shifts and a weekly pay range of $1,505 to $1,589. Work in a skilled nursing facility / nursing home setting with a census around 25 beds.

Responsibilities
- Administer prescribed medications and treatments per physician orders; monitor responses and document in the charting system
- Monitor vital signs and perform routine assessments; escalate changes to the RN or physician as needed
- Provide skilled nursing care for adults/geriatrics, including wound care, ADLs, comfort measures, and behavior management
- Obtain and interpret 12-lead ECGs; respond to cardiac status changes and communicate findings to the care team
- Maintain accurate, timely documentation in EPIC; coordinate with the interdisciplinary team (PT, Radiology, Social Services, Pharmacy) to support individualized care plans
- Ensure safety and infection control, follow fall precautions, and assist with float within skillset as required

Requirements
- Active New York single-state LPN license in hand (not compact)
- BLS certification
- Minimum 1 year nursing experience; Nursing Facility / Nursing Home experience required; LTAC experience listed as required; previous charge experience; travel experience required; first-time travelers accepted
- EPIC charting experience required
- Ability to work in a skilled nursing facility and float within skillset
- Weekend requirement: every other weekend; Holiday requirement: available every other holiday, if needed
- RTO: cannot submit candidates with more than 10 days of requested time off
- COVID-19 vaccine: Not required
