Overview
Licensed Practical Nurse (LPN) – Long Term Care | Clifton Springs, New York | Travel assignment starting 08/11/2026 for 13 weeks | Weekly pay range: $1,366–$1,411 per week | No guaranteed hours listed | Shift: 5 days per week, 8-hour evenings (1500–0330) | Setting: long-term care / skilled nursing unit within a healthcare facility

Responsibilities
- Provide direct nursing care to adults and geriatric residents in a long-term care setting, following individualized care plans
- Administer medications (oral) and perform IM/SQ injections; monitor vital signs and glucose as needed
- Manage feeding tube care (G/NG), and perform Foley catheter care for male and female patients
- Administer oxygen therapy and monitor respiratory status; respond to changes in patient condition
- Document care activities in EPIC; support admissions, status changes, and interdisciplinary communication
- Collaborate with interdisciplinary team ( Respiratory, Radiology, Social Services, Pharmacy ) and assist with admissions and discharge processes

Requirements
- Active New York single-state LPN license (not a compact license)
- BLS certification
- Minimum 1 year of nursing experience; prior charge nurse experience required
- Travel nursing experience required; LTAC/long-term care experience required
- EPIC charting experience is mandatory
- Ability to work the stated shift (1500–0330) and float within your skillset as needed
