Home Health Physical Therapist

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you’ll empower individuals to overcome limitations and enjoy a better quality of life. This role includes a $10,000 sign-on bonus. Responsibilities Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function. Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with the physician’s Plan of Treatment and the goals of the patient team. Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. Document patient observations, interventions, and evaluations promptly and thoroughly. Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. Qualifications Required: Current and unrestricted Physical Therapy license Required: Current CPR certification Required: Experience collaborating with a team of healthcare professionals Preferred Qualifications One year of experience as a Physical Therapist Pay and Benefits Pay per visit/unit range: $76.00 – $106.00 Annual base pay range: $111,600 – $153,600 Description of Benefits: Medical, dental and vision plans; 401(k); paid time off; short-term and long-term disability; life insurance; and more. Additional Information TB Statement: This role is patient-facing and may be required to participate in Humana’s TB screening program. Driving Statement: Requires a valid state driver’s license and personal vehicle liability insurance; maintain coverage per state requirements. Schedule Full-time; 40 hours per week.
