Clinical Pharmacist

Job Summary As a Clinical Pharmacist, you will work with the healthcare team at Mercy to provide safe and effective drug therapy. You will serve as a drug information resource for pharmacy, nursing, and medical staff, help optimize medication regimens, and ensure medication‑safety processes across the institution. Key Responsibilities Verify and enter medication orders accurately to support timely drug availability and adherence to policy. Review prescriptions, medication orders, and patient charts for appropriateness, potential interactions, and therapeutic outcomes. Consult with physicians, nurses, and other pharmacists to recommend therapeutic drug choices and dosing adjustments. Maintain accurate patient pharmacy records and documentation of clinical interventions. Participate in medication‑therapy rounds and multidisciplinary rounds to provide clinical expertise. Provide drug information and counsel to healthcare professionals and patients as needed. Ensure compliance with regulatory standards, including Joint Commission accreditation requirements. Qualifications Doctor of Pharmacy (PharmD) from an accredited college of pharmacy. Valid, unrestricted pharmacist license in the state of employment. Preferred: at least 2 years of clinical pharmacist experience in a hospital or acute‑care setting. Strong verbal and written communication skills; ability to advise and collaborate with interprofessional teams. Demonstrated critical‑thinking, decision‑making, and problem‑solving abilities under minimal supervision. Commitment to patient safety and effective medication use. Compensation Annual salary: $95,619.94 (full-time). Additional positions within the same organization offer hourly rates ranging from $52.00 to $70.00 depending on experience and location.
