Ambulatory Telehealth Registered Nurse (RN), Neurosurgery, Full Time, First Shift

Job Summary UC Health is hiring a full‑time Ambulatory Telehealth RN to coordinate patient care in an ambulatory setting. The RN will provide direct patient care, develop care plans, and collaborate with interdisciplinary teams to optimize patient outcomes. Approximately 60 % of the time is patient‑facing, with the remaining time for administrative duties and quality improvement. Location Cincinnati, OH, United States Qualifications Minimum required: Associate's Degree in Nursing. Preferred: Bachelor's Degree in Nursing. Current Registered Nurse license in the state of Ohio. Basic Life Support (BLS) certification preferred. Minimum of 2 years of nursing experience in an ambulatory or outpatient setting. Responsibilities Engage in population‑appropriate communication. Provide clear instructions to patients/families regarding treatment. Involve family/guardian in assessment, initial treatment, and continuing care. Identify physical limitations of the patient and deploy intervention when necessary. Recognize and respond appropriately to patients/families with behavioral health problems. Interpret population‑related data and plan care appropriately. Respond to medical needs regarding unique psychosocial needs, religious or cultural norms. Perform treatments, administer medication, or operate equipment safely. Recognize and respond to signs/symptoms of abuse or neglect. Patient Assessment & Interventions Act as an expert clinician and role model in providing quality patient care. Utilize the nursing process for direct patient care. Conduct thorough assessments of patients’ health status during clinic visits and re‑evaluate throughout the visit. Document nursing history and assessment; initiate referrals based on patient needs. Collect basic and advanced patient data; monitor trends that could impact health. Maintain awareness of comfort and safety needs during ambulatory appointments. Respond to life‑saving techniques per standards, policies and procedures. Provide accurate hand‑off reports when patients move between ambulatory and inpatient areas. Recognize and report changes in patient condition and abnormal laboratory results. Perform point‑of‑care testing assessments as needed to support clinical well‑being. Initiate and implement individualized patient care plans in collaboration with other providers. Develop and utilize patient care pathways that ensure consistent quality of care. Demonstrate competency in administering medications and wound care. Complete nurse visits and document timely and accurately. Assist with minor procedures and diagnostic tests, ensuring patient comfort. Monitor patients for changes and report findings to the healthcare team. Recruit patients for studies. Assist staff nurses in quality assurance and use of standard documentation. Meet new patients, introduce self, and explain role. Establish priorities of care based on assessments and patient needs. Apply critical thinking to evaluate and implement physician orders. Ensure nursing documentation is accurate and timely. Maintain a professional, calm demeanor throughout all interactions. Patient Education Meet with patients and families to identify educational needs. Educate patients and families about health conditions, treatment plans, medications, and self‑care techniques. Reassess understanding and re‑educate as needed. Use current nursing literature and teaching techniques to guide patient education. Act as expert patient educator for disease‑based and clinic‑based care teams. Document teaching sessions and patient responses. Facilitation Of Care Coordinate referrals, schedule follow‑up appointments, and ensure timely access to services. Communicate effectively with patients regarding test results and treatment plans. Address patient concerns and provide guidance throughout treatment. Anticipate changing patient needs and act proactively to reduce delays and financial toxicity. Participate in interdisciplinary team meetings to discuss patient care. Collaborate with social workers, dietitians and others to address comprehensive needs. Liaise with other departments and organize interdepartmental projects. Participate in transitions of care planning and document care plans. Support prior authorization for medication and diagnostic tests. Leadership & Professional Development Serve on nursing and hospital‑wide committees that influence patient care. Assist in developing and revising policies for nursing practice. Implement evidence‑based practice unit‑wide. Collaborate on performance improvement initiatives system‑wide. Promote accountability and commitment to patient care standards. Serve as resource within specialty. Lead orientation and education programs. Pre‑mentor nursing students, interns and other healthcare professionals. Stay current in research and clinical nursing practices. Participate in and contribute to staff meetings. Compliance & Safety Maintain confidentiality of patient information. Understand regulatory requirements and accreditation standards. Monitor safety policies and procedures. Collaborate with multidisciplinary representatives to share safety best practices. Demonstrate knowledge of legal and regulatory issues (HIPAA, EMTALA, etc.).
