Key Responsibilities\n

• Conduct thorough assessments and develop individualized treatment plans based on the evolving needs of each patient.

• Engage in regular communication with treatment team members, family members, and other stakeholders to discuss patient progress and adjust care strategies accordingly.

• Maintain detailed administrative records, including monthly and quarterly progress notes, to effectively track treatment outcomes and ensure compliance with legal and ethical standards.

• Organize and conduct home visits and in-person meetings, as necessary, to complement teletherapy efforts and gain insights into patient environments.

• Provide training and support to team members regarding program procedures and documentation to ensure consistent and effective service delivery.

• Actively participate in interdisciplinary team meetings, contributing valuable insights and fostering collaboration across different specialties.

Patient Population / Client Focus\n

The LPC will work with a diverse range of clients including adults, adolescents, and families facing challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
