Admissions Coordinator

QUALIFICATIONS Candidate must possess 1-2 years prior experience in long-term care setting or related field. The admissions Coordinator represents Island Nursing and Rehab Center to the community and markets the facility to others. Employees must possess knowledge of Medicare/Medicaid and understanding of PRIs, federal and NYS regulations, and reimbursement systems. Must possess excellent communication skills, cooperate with others, develop relationships with prospective families and residents, and coordinate admissions department operations. JOB SUMMARY Provides information to families and referral sources regarding admission criteria and process. Performs secretarial duties to support administrative details and ensures completion of paperwork related to admission, transfer, and discharge of residents. RESPONSIBILITIES Receives incoming referrals and calls from hospitals, community agencies, and families; logs referrals into the CRM and initiates financial and clinical reviews. Interviews prospective residents and families, provides orientation, tours of the facility, and explains the application process and current New York State regulations. Provides resident and family with admission agreement contract; receives signed agreement and maintains it in the facility EMR (PCC). Maintains communication with hospitals and other agencies for marketing and follow-up on current referral applications. Coordinates the intake application process, verifies insurance, and assists in obtaining MCO authorizations. Controls various files and statistics relating to applications, admissions, discharges, and bed holds; completes monthly statistics reports as requested. Completes and distributes facility’s daily census report through the EMR, PCC dashboard, and daily midnight census. Notifies hospitals, nursing homes, and families of bed availability when admissions are required. Schedules and performs facility tours of potential admissions. Answers telephone information requests with courtesy, accuracy, and respect for confidentiality; documents and reports clearly and concisely. Demonstrates knowledge, respect, and sensitivity to resident rights as identified in the Resident Bill of Rights and facility policy and mission statement; assures confidentiality of all resident information. Attends in‑service education as required. Embraces the Central admissions model and identifies resident needs within the Gurwin HealthCare System operations.
