Job Title Registered Nurse (RN) Home Health – Residential Home Health and Hospice – Graham Healthcare Group – Swartz Creek, Michigan Overview Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health care in Swartz Creek, Michigan. The role is ongoing, full-time 40 hours per week with 8‑hour shifts. Responsibilities Lead and collaborate as a home health registered nurse case manager to manage and deliver patient care plans. Support patients in Schwartz Creek and surrounding areas. Participate in a 4‑month orientation pay period and clinical outcome bonus program. Maintain critical deadlines for charting and use of EMR systems. Qualifications Registered Nurse license in the state you work. Graduate from an approved school of practical nursing. One year of RN experience and clinical competence to deliver quality patient care. Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting. Ability to self‑manage time and schedule. Demonstrated experience with tablets, mobile phones and EMR software.
