Patient Services Coordinator

Job Title Home Health Scheduling Coordinator - HCHB & Every Weekend Required Location Remote/Nationwide, USA Employment Type Full‑time Remote Schedule 40 hours per week. Must work every Saturday and Sunday 8:00AM–5:00PM EST or CST. Remaining weekdays are flexible. Compensation $20.00 – $24.00 per hour (fixed maximum). Compile daily schedules of clinical staff. With the direction of the Assistant Clinical Manager, assign POD clinicians to patients. Assist POD personnel in care coordination of patient/client services. Serve as a liaison between field staff, patients/clients, and POD personnel. Communicate with CFSS when order approval is holding scheduling. Complete all tasks/workflow daily and communicate with ACM regarding any workflow that cannot be processed before shift end. Process appropriately all visits that have been sent back from clinicians. Participate in weekend rotation as needed. Qualifications Associate’s degree preferred. At least one year of experience in home health (preferred). At least one year of experience in a customer service capacity. Proficiency in Microsoft Office suite. Minimum of two years general office experience, with at least one year of data entry or word processing functions. Previous home health experience preferred.
