How You'll Make An Impact In This Role

• Schedule medical appointments, referrals, and specialized services with outside healthcare providers for assigned patient populations.
• Coordinate schedule changes and cancellations, securing required approvals in accordance with site-specific protocols.
• Enter patient information, appointment details, and special instructions accurately into the electronic medical record (EMR) system.
• Maintain up-to-date provider contact information and complete required documentation to ensure continuous regulatory compliance.

What Minimum Requirements You'll Need

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
