· Ensure consumer clients needs are been met at all times by assigned personnel.

· Ensure Consumer Care plan is followed per order.

· Communicate with consumers weekly to get input on how care is progressing.

· Ensure compliance with State and Federal requirements regarding service provision are addressed and followed to the book.

· ON-CALL responsibilities to ensure continuity of services.

· Address and resolve all consumer service related issues that arise.

· Ensure Consumer care is properly documented and recorded properly.

· Ensure the services system software applications are updated daily.

· Ensure Consumers are receiving services in accordance to issued service order.

· Ensure personnel time worked is documented and communicated to payroll and other related department timely.

· Scheduling of appropriate skilled persons to fill the consumer assignment, input the information in the system and coordinate all areas of scheduling.

· Close communication between consumer, caregiver, case managers and office personnel.

· Weekly staffing and concerns report and presented to administrator every Friday.

· Maintain and update Consumer and Caregiver database.

· Communicate hospitalizations or any Consumer issued with their case managers.

· Keep a weekly consumer Census.

· Call 15 consumers weekly to enquires about the services we are providing. Are they happy

· Visit one consumer weekly.

· Collect and verify all caregiver notes and Time slips bi weekly to ensure care and payroll compliances.

· Ensure to double check on caregiver credentials.

· Assist with orientation and in services of caregivers.

· Participate in personnel policies revisions ( when requested )

· Maintain strict professional ethics, confidentiality and privacy of patients and or their family members.

· Daily scheduling report.

· Address service order issues with Service Coordinators.

· PERFORM OTHER DUTIES AS ASSIGNED BY SUPERVISOR, DON AND OR THE ADMINISTRATOR.
