Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical functioning, as well as social and mental function, aligning with department objectives.
- Deliver evidence-based interventions for diverse patient groups and monitor response to therapy.
- Document patient progress, update treatment plans, and communicate results to the interdisciplinary team.
- Serve as co-leader in an interdisciplinary approach to patient groups, facilitating coordination among therapists, physicians, and other care professionals.
- Support discharge planning and patient education to promote functional independence and sustained outcomes.

Requirements
- Doctor of Physical Therapy (DPT) or equivalent professional degree
- Current California physical therapy license or eligible for licensure
- Outpatient physical therapy experience preferred
- Ability to work a daytime 4x10-hour schedule in an outpatient care setting
- Strong communication and teamwork skills, with a patient-centered approach
