Bilingual Field-Based Care Manager (Medicaid)

CareCollab is seeking a field-based Care Manager to coordinate services for Medicaid members with intensive needs in New York City. You will conduct comprehensive assessments, develop care and crisis plans, and collaborate with healthcare, housing, behavioral health, and social services partners.
