Key Responsibilities\n

• Conduct thorough and ongoing assessments to accurately determine the mental health needs of each patient.

• Regularly communicate with supervisors, direct care staff, and family members to provide updates on patient progress and collaborate on care strategies.
\n
• Develop and provide a detailed schedule of appointments, home visits, and observations to enhance treatment monitoring.
\n
• Organize and coordinate home visits to facilitate comprehensive evaluations, including chart reviews and staff interviews.
\n
• Ensure that the program plan is effectively implemented, regularly assessing its impact on patient care.
\n
• Participate actively in Interdisciplinary Team Meetings and contribute to discussions on patient care solutions.
\n
• Document patient progress through timely monthly and quarterly notes.
\n
• Lead the training of staff and team members to ensure they are well-equipped to apply the program plan and maintain accurate documentation.
\n
• Conduct follow-up interviews and home visits to provide continued support and assess the ongoing needs of individuals.
\n
• Maintain organized and accessible records for easy retrieval and sharing with authorized parties.
\n
• Act as the primary liaison between the organization and the broader mental health community to facilitate collaborative development goals.

• Assist in the development of training materials aimed at enhancing program efficacy and staff performance.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Reliant Health Center, you will serve a diverse range of patients, including both adults and youth experiencing various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon is essential.

• A minimum of two years of experience in a counseling capacity, preferably within a clinical environment.
