Work Environment\n

This role will be conducted primarily in a remote setting, providing you with the flexibility to operate from home while still participating in team meetings and case discussions through virtual platforms. You will manage your schedule around client needs, which may occasionally require evening or weekend support to ensure urgent matters are addressed promptly.

Key Responsibilities\n

• Conduct thorough assessments of clients' social, emotional, and environmental challenges, utilizing established screening tools and clinical judgment.

• Develop individualized service plans that encompass specific goals, intervention strategies, and progress evaluation, adapting them as client circumstances evolve.

• Provide empathetic counseling and support through one-on-one and family sessions, targeting issues related to grief, loss, and emotional distress.

• Collaborate with clients to help them access community resources, including financial assistance, healthcare services, and mental health support, ensuring a holistic approach to care.

• Offer immediate crisis intervention and risk assessment services as necessary, developing tailored safety plans in partnership with clients.

• Engage with a broad network of community organizations to coordinate services and advocate for clients, ensuring they receive comprehensive care.

• Participate in community outreach initiatives to promote agency services and foster awareness about social issues faced by our client populations.

• Attend case conferences and staff training sessions to promote continuous learning and improve service delivery.

Patient Population / Client Focus\n

Your work will primarily involve serving individuals and families navigating the emotional and psychological impacts of serious illness, as well as those requiring support during hospice care. This includes addressing varied challenges such as grief over loss or anticipatory grief in terminal conditions, ultimately aiding clients in finding peace and understanding as they face life’s most profound transitions.

Qualifications\n

• Master's degree in Social Work from a regionally accredited program.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

• Proficiency in trauma-informed care approaches and familiarity with social and economic justice issues relevant to our client populations.

• Exceptional communication, cultural competence, and problem-solving skills to effectively engage with clients from diverse backgrounds.

Preferred Experience\n

Compensation and Benefits\n
