Key Responsibilities\n

• Conduct comprehensive assessments to identify the social, emotional, and environmental challenges faced by clients, particularly those nearing the end of life.

• Develop personalized service plans that include goals, interventions, and progress monitoring to assist clients and their families throughout the hospice experience.

• Provide counseling services tailored to individuals and families, helping them navigate emotional struggles, grief, and other life transitions related to serious illness and end-of-life care.

• Collaborate effectively with clients to connect them with essential community resources and support services that will enhance their quality of life.

• Offer critical crisis intervention, risk assessment, and safety planning as needed to ensure the well-being of clients in distress.

• Work alongside community agencies and organizations to coordinate services and advocate effectively for clients’ needs, ensuring they receive the care and support they deserve.

• Engage in community outreach to raise awareness of hospice services, promoting education regarding end-of-life issues and available support.

• Participate in case conferences and team meetings to share insights, strategies, and updates regarding client progress and ongoing needs.

Patient Population / Client Focus\n

The primary focus of this role is to support patients receiving hospice care, along with their families. This population often experiences complex emotional needs arising from terminal illnesses and the emotional toll surrounding end-of-life decisions. The LCSW will provide critical emotional support while facilitating communication between patients, families, and the healthcare team to ensure a seamless care experience.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n

The offered salary for this position ranges from USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
