Key Responsibilities\n

• Provide empathetic counseling and emotional support to patients and their families, addressing adjustment challenges and promoting emotional well-being throughout the rehabilitation process.

• In collaboration with a team of rehabilitation professionals, develop individualized treatment plans that address the unique psychosocial needs of each patient.

• Guide patients in accessing valuable community resources such as support groups and vocational rehabilitation programs, ensuring they have tools to succeed post-recovery.

• Educate patients and their families on community resources and post-rehabilitation services that can further support their reintegration into everyday life.

• Contribute to discharge planning by facilitating safe and effective transitions to home or other care settings, documenting progress and follow-up requirements.

Qualifications\n

Compensation and Benefits\n
