License: NYS Nursing Assistant Certification required

Certifications: BLS (AHA) required

Must-Have: High School Diploma or GED

Description: Meet residents' Activities of Daily Living (ADLs) needs as outlined in the plan of care, ensuring compliance with policies and procedures within a safe and comfortable environment. Assist with hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Complete documentation per protocol for all care provided. Maintain residents' environment and safety as outlined in the plan of care, including prompt response to call lights and alarms. Ensure provision of safety mats and low beds, appropriate use of restraints under the direction of Nursing, and maintaining cleanliness of the unit. Monitor behavior and fall prevention, and ensure effective use of assistive devices and equipment. Document completion of all safety measures per protocol. Meet residents' nutritional needs as outlined in the plan of care, including meal delivery, tray preparation, and adaptive equipment usage. Honor residents' preferences and choices while providing feeding and nourishment as needed. Complete documentation per protocol for all nutritional support provided. Meet residents' rehabilitative needs as outlined in the plan of care, including range of motion exercises, ambulation, transfers, and maintaining proper body alignment. Document completion of all rehabilitative tasks per protocol. Report residents' rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely manner. Identify and report changes in residents' physical, emotional, and psychosocial status to the Charge Nurse promptly. Properly handle occupational exposure to blood-borne pathogens, as tasks may involve potential contact with blood, body fluids, or tissues.
