Work Environment

This position is predominantly onsite, allowing Physical Therapists to work in the homes of patients within the Rochester area. Key Responsibilities

• Manage the complete therapeutic care process for a designated group of patients in coordination with physicians and the broader healthcare team.
• Conduct thorough initial assessments to identify patient needs and develop customized care plans that emphasize patient engagement and outcome success.
• Implement therapeutic interventions through specialized techniques and clinical practices that align with physician directives.
• Provide education and instruction to patients, families, and caregivers regarding therapeutic exercises, use of assistive devices, and recommended home programs to maximize rehabilitation success.
• Prioritize patient caseloads to ensure efficient delivery of care while adapting to changing patient conditions and staffing needs.
• Monitor patient progress, adjusting treatment plans as necessary to ensure therapeutic objectives are being met.
• Document clinical findings and treatment strategies in compliance with applicable healthcare regulations and organizational policies.

Patient Population / Client Focus

The primary focus of this role is on patients receiving home care services, including both adult and pediatric populations. Physical Therapists will work with individuals recovering from surgery, managing chronic conditions, or rehabilitating after injuries. By providing care in their own home environments, therapists can assess functional capabilities and tailor interventions that directly address patients' lifestyle and recovery goals. Fostering strong, trustworthy relationships with patients and their families is essential to achieving shared health objectives.

Qualifications

• A valid license as a Physical Therapist in New York State.
• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Cardiac Life Support (BCLS) certification, or willingness to acquire within 30 days of employment.
• A strong commitment to patient care and the ability to communicate effectively with patients, families, and healthcare team members.

Preferred Experience

• Prior experience in a home health setting is advantageous but not required.
• Experience working with orthopedic and surgical rehabilitation patients is a plus.
• Familiarity with electronic medical record (EMR) systems used for documentation and patient management.

Compensation and Benefits
