This role is primarily onsite in a home care setting, where you will directly engage with patients in their own environments, making tailored assessments and interventions possible. The facility encompasses a state-of-the-art, 5000-square-foot sports medicine center outfitted with advanced rehabilitation technologies, providing a conducive atmosphere for team collaboration and patient care. Key Responsibilities

• Evaluate and establish individualized care plans, identifying both the objectives and requirements specific to each patient's health needs.
• Deliver personalized, evidence-based treatments that facilitate movement and enhance functional abilities, managing a focused caseload of one patient per hour.
• Develop and conduct exercise regimens while employing manual therapy techniques to ensure optimal rehabilitation results.
• Document patient progress diligently, continually assessing treatment effectiveness and reporting on outcomes to refine ongoing care strategies.
• Motivate and guide patients through their rehabilitation processes to foster independence and self-assurance in their movement capabilities.
• Collaborate with an interdisciplinary team of healthcare professionals to create comprehensive care plans that optimize overall patient care.
• Advise patients on lifestyle modifications that promote health and well-being, focusing on enhancing strength, flexibility, balance, and coordination.
• Minimize reliance on pharmacological interventions by offering solution-focused alternatives to surgical procedures.

The ideal candidate will primarily engage with individuals covered under private medical insurance, who are seeking to improve their physical health and mobility capabilities. Our patient demographic often includes those recovering from orthopedic surgeries or injuries, as well as individuals dealing with chronic conditions. Qualifications

• A Doctorate in Physical Therapy (DPT) is required for consideration.
• Active licensure as a Physical Therapist within the state of New York.
• A clean professional record with no board actions or disciplinary measures.
• Proven experience with manual therapy techniques and conducting individualized exercise programs.
• Familiarity with Electronic Medical Records (EMR) software, specifically platforms such as WebPT.

Preferred Experience

• Experience in an outpatient orthopedic setting is highly desirable for success in this role.
• Prior exposure to home care environments or working directly with patients recovering from surgeries will be an added advantage.

Compensation and Benefits

The position offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience and qualifications.
