Chronic Care Management (CCM) Specialist Job Summary We are seeking a compassionate and dedicated Care Specialist to join our team. Responsibilities Conduct comprehensive assessments of patients with chronic conditions, including gathering medical history, understanding the patient’s social determinants of health, and identifying care needs. Monitor patient health status through regular phone calls or texting and report any changes or concerns to the appropriate healthcare professionals. Maintain accurate records of patient care and progress. Participate in ongoing training and professional development opportunities. Educate patients on disease management, lifestyle modifications, self‑monitoring, and medication adherence to empower them to take an active role in managing their health. Maintain accurate and up‑to‑date documentation in the electronic health record (EHR) system. Track patient outcomes and progress towards care goals and document any changes or updates to care plans. Qualifications Associate’s or Bachelor’s degree in Public Health, or a related field. Previous experience in a caregiving role is highly desirable. Strong communication skills, both verbal and written. Ability to work independently as well as part of a team. Compassionate nature with a strong commitment to providing high‑quality care. Flexibility to work various shifts, including evenings and weekends as needed. Excellent organizational and time management abilities.
