Position Summary

The Clinical Support Specialist plays a vital role in supporting the  clinical operations of a home health agency. This position provides direct assistance to the clinical team, helping ensure  accurate documentation ,  regulatory compliance , and  effective coordination of patient care . The role works under the clinical oversight of the RN Clinical Manager and Administrator and practices within the Texas LVN scope of practice. The ideal candidate is detail-oriented, tech-savvy, and experienced in home health workflows, particularly related to  EMR systems, OASIS documentation, and scheduling support .

Key Responsibilities

Clinical Documentation & EMR Support

• Review and audit clinician documentation for accuracy and completeness (e.g., OASIS, Plan of Care, visit notes).
• Track, flag, and follow up on documentation deficiencies or missing signatures.
• Assist with uploading, organizing, and maintaining accurate clinical records in the EMR system.

Coordination & Communication

• Support field staff with real-time clinical or scheduling questions.
• Serve as liaison between clinicians, intake, QA, billing, and administrative departments.
• Communicate with physicians' offices to obtain signed orders and clarify care plans.

Regulatory & Compliance Support

• Monitor compliance with Medicare Conditions of Participation and agency protocols.
• Assist in preparing for surveys or audits by ensuring clinical records are complete and accurate.
• Support the QAPI program through data collection and tracking of clinical indicators.

Training & Technical Support

• Provide EMR guidance to new or existing field staff.
• Assist with onboarding processes for new clinicians regarding documentation workflows.

Qualifications

Education & Licensure:

• Current, unrestricted Texas LVN license 
• Current CPR certification.
• Valid driver's license, reliable transportation, and automobile insurance when travel is required.

Experience:

• 1–2 years of experience in home health, healthcare administration, or medical office setting.
• Strong working knowledge of  EMR systems
• Familiarity with  OASIS, Plans of Care, Medicare documentation standards a strong plus.

Skills:

• Excellent organizational and communication skills.
• Strong attention to detail and ability to multitask in a fast-paced environment.
• Proficiency in Microsoft Office (Excel, Outlook, Word).
• Ability to work both independently and as part of a team.

Working Conditions
