Location: St. Paul's PACE Corp, 3444 Camino Del Rio North, San Diego, California, United States of America Job Description Posted Thursday, July 2, 2026 at 10:00 AM San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals 55 years or older who are living with chronic illness or disabilities and need coordinated medical care to continue living independently in their home and community. Typical schedule for this position is 8:00 am to 4:30 pm with varying days and availability including weekends. The position also participates in an on‑call rotation. Job Summary: Under the direction of the Clinical Services Supervisor, the IHS Staffing Coordinator ensures PACE participants receive their IHS services according to their plan of care (POC) by coordinating schedules of IHS employees and caregivers, assigning caregivers to participants daily, organizing efficient driving routes, ensuring clear communication, and ensuring IHS staff follow assigned schedules and service orders. Duties & Responsibilities Attended Interdisciplinary Care Team (IDT) and POC daily meeting. Schedule IHS services for all PACE participants and respond appropriately to needs identified by the IDT. Coordinate IHS services so they are timely and efficient. Prepare caregiver schedules and modify, cancel, or reschedule as needed, adapting to unanticipated changes, and communicating changes to affected personnel in a timely manner. Review and audit daily visits and prepare reports. Promote good customer service by maintaining positive relationships with PACE participants, families, caregivers, colleagues, and staff. Maintain adequate records for IHS services and ensure policies and procedures are communicated and followed. Report all issues promptly with possible solutions to Management. Perform other duties as assigned. Qualifications One year previous scheduling or administrative experience preferred.
