Field Clinical Assessor - Medicaid PCS

Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. The role focuses on completing needs-based eligibility determinations for Medicaid-funded PCS provided in home or in adult care or supervised living homes, and assessing level of care to help individuals stay in or return to home- and community-based settings.
