Field-Based RN Clinical Assessor PCS Eligibility

Acentra Health

Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. The purpose of this position is to complete needs-based eligibility determinations for Medicaid-funded personal care services in home or in adult care or supervised living settings, and to assess level of care to keep individuals in home-based environments. This role requires a valid driver’s license and travel to various locations, with driving up to 8 hours per day and as many as 120 miles per day. #J-18808-Ljbffr Acentra Health
