Clinical Assessor, RN — Field Medicaid PCS Evaluations

Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. The role conducts needs-based eligibility determinations for Medicaid-funded personal care services, and assesses level of care to keep individuals in home- and community-based settings.
