Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to determine needs and create individualized treatment plans.

• Engage with families during the development and implementation of Individualized Family Service Plans (IFSP), ensuring that their preferences and concerns are prioritized.

• Monitor and evaluate child progress as a collaborative team member, adjusting interventions as necessary to promote optimal developmental outcomes.

• Deliver educational support and coaching to families, equipping them with skills and strategies to enhance their child's growth in key areas such as physical, cognitive, and social-emotional development.

• Utilize evidence-based practices and natural learning environment approaches in interventions, promoting development in real-world settings.
\n
• Ensure compliance with all relevant federal and state regulations, maintaining the highest standards of care and confidentiality.

• Foster strong, trusting relationships with children, families, and team members to facilitate a supportive learning atmosphere.

• Document all services and interventions accurately in accordance with clinic policies and funding requirements.

Patient Population / Client Focus\n

This role focuses on providing support to the youngest members of our community, particularly children with disabilities or developmental delays from birth to three years of age. You will work closely with families to help children acquire essential skills in a nurturing and individualized manner. Our services extend beyond just therapy; we aim to involve parents and caregivers in every step of the intervention process, fostering a holistic approach to child development.

Qualifications\n

• Required: Master’s Degree or Doctorate in Social Work.

• Must hold a valid LCSW license from the Department of Public Health.

• A valid driver’s license is necessary for occasional travel to client homes or community locations.
