POSITION SUMMARY:
The Client Coordinator is responsible for ensuring the overall operations and services re being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The Client Coordinator serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan.
The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. The Client Coordinator is responsible for ensuring service provision by all disciplines represented on the treatment team. The Client Coordinator meets routinely with other Team Clinicians for coordination and integration of services. The Client Coordinator coordinates with the Program Coordinator(s) and staff and the provision of individual and day program services as well as the maintenance of a therapeutic milieu which is conductive to participants’ health and well-being.
The Client Coordinator coordinates and oversees the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department and completion of a required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. The Client Coordinator accepts primary responsibility for development of program goals and methodologies and for supporting the DSPs to correctly implement and document such activities. The Client Coordinator is primarily responsible for ensuring the goals and activities are developmentally appropriate and are in response to identified prioritized needs, and to relay relevant information to the team.
The Client Coordinator ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration, coordinating service delivery and planning activities with day program services, monitoring service provision by agency clinical staff, consultants and community based vendors, and ensuring services are in accordance with IDT treatment recommendations.
The Client Coordinator ensures provision of a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to ensure provision of coverage levels consistent with the treatment needs of the individuals and regular monitoring of the day program setting to ensure maintenance of an atmosphere conducive to individuals' development and well-being, where the rights of each individual are upheld and respected. The Client Coordinator regularly records progress of individuals and documents all changes in Plans of Care / ISPs according to procedures established by the agency.
The Client Coordinator reports regularly to the Supervisor on issues of program compliance, noncompliance, and remedial actions taken to correct deficiencies. The Client Coordinator identifies needs and requests supports, assistance or direction to meet service needs and ensure program compliance. The Client Coordinator participates in clinical reviews, incident review and human rights committee meetings and other meetings as directed by Supervisor. As schedule allows, the Client Coordinator is the primary chair on behalf of the agency in the Life Plan Meetings. The Client Coordinator reviews documentation and provides technical assistance or direction to program management and staff.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Employee must possess the ability to interact with individual and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner. Previous experience working with individuals with IDD/Mental Illness is required and proven experience managing teams. Competency in Microsoft applications including Word. Previous experience in a Day Support Program for individuals with an IDD or Special Education teaching/learning environment is preferred. Previous experience in the care, training, habilitation and development of individuals with an IDD or IDD/mental illness. Employee must possess the ability to interact with individual and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner.
Must have a passion for and desire to work with people with intellectual disabilities and other physical or mental disabilities. Knowledge of basic human physical and psychological needs. Knowledge of simple health and safety precautions. Knowledge of simple nursing care, first-aid, behavior management, personal and environmental hygiene. Ability to write progress and behavior reports using a professional, objective style and voice. Ability to use computers and other electronic equipment. Ability to interact with individuals, community members and other treatment team members. Ability to read and comprehend technical training manuals. Ability to follow written and oral instructions course of action. Ability to learn basic operation and maintenance of adaptive equipment. Ability to be supportive and patient. Ability to be a team player. Ability to observe, record and report participant’s behavior, attitude, and physical condition. Ability to prioritize conflicting deadlines. Ability to observe, record, and report participant’s behavior, attitude, and physical conditions. Ability to obtain First Aid/CPR certification and Medication Administration Training.
ESSENTIAL RESPONSIBILITIES:
- Maintain all individual Day plans and update each plan as needed.
- Attend all Individualized Service Plan meetings, and applicable reviews.
- Develop goals for the individual, ensure individual’s progress, review such progress daily, and trend individual progress data on a monthly basis, documenting all outcomes and make appropriate recommendations.
- Responsible to assist in the individual and his or her advocate in obtaining all services outlined in the Day Habilitation Plan.
- Responsible to complete monthly progress notes for all participating individuals.
- Maintain all individual Habilitation plans and update each plan as needed.
- Identify needed trainings for Direct Support Professionals to ensure adequate services.
- Responsible for handling all issues with the parents of the individuals assigned.
- Ensure that issues with parents, family, advocates or residential staff are communicated with management.
- Conduct frequent and routine program observations to observe goal implementation and individuals’ response to services.
- Conduct compliance reviews of the program to ensure state and agency record and program compliance and monitors and reviews all general events and incidents and develops systems to ensure the health and safety of individuals. Identifying risks and developing corrective plan to avoid potential incidents. Review all records and documentation prepared by the Client Coordinators on a regular basis to ensure compliance.
- Maintaining program records and ensuring compliance ongoing, which include but not limited to Life Plans, Staff Actions, IPOP, Assessments, Level of Care and DDP2. In addition to ensuring these plans and records are consistent with other clinical service plans, such as PONS and BSP.
- Evaluation of individual's strengths needs and develop Habilitation Plans, designed to develop personal and life skills.
- Participate in the development of Life Plan in relations to the day program services.
- Develop and supervise the implementation of the Staff Action Plan (SAP)/Day program Habilitation Plan (DHP) goals and the Individualized Protective Oversight Plan (IPOP)
- Monitor the progress of individuals and write the monthly progress notes.
- Review all Interdisciplinary Team summaries/ reports and recommendations to ensure implementation and follow up.
- Ensure timely completion of Client Coordinator summaries for monthly, semi-annual and annual reviews. Submit semi-annual and annual reviews to Care Managers for inclusion in the Life Plan.
- Attend Life Plan (Individualized Service Plan) meetings (Annual review and 6-month review), monthly staff meetings, and Interdisciplinary Team (IDT) meetings
- Maintains effective communication and adequate follow up with individuals’ teams, staff and family for quality continuity of care and qualified services
- Complete addendum/changes to Day program Habilitation Plans as per team agreement.
- Maintain contact with the individual's family, Care Manager, Behavior Intervention Specialists, Nurses, Residence, treatment centers, and other agency programs to ensure continuity of services
- Review and update annually and as needed, the IPOP, Assessment and other plans according to the individual's needs, strengths and necessary supports. Ensure timely completion of all annual evaluations/ assessments by various professionals.
- Participate in the internal audit process. Review deficiencies with IDT and follow-up with appropriate parties.
- Ensure that all Life Plans, Staff Actions, Monthly Notes, Daily Service Documentation data is complying according to OPWDD and OMIG requirements.
- Responsible to review the weekly billing, approve and submit service documentation for billing processes.
- Notify team members, parents, guardians, relatives and day programs of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.
- Provide training to staff on Day program Habilitation Plans to ensure the appropriate implementation of plans
- Along with the Program Coordinator, assist with leading team meetings.
- Along with the Program Coordinator, assist with admissions and discharge procedures; such as team meetings, evaluation, program documentation, etc.
- Coordinates and ensures the overall development, periodic evaluation, and review of each individual's individual plan of care and supervises the delivery of training and rehabilitative services relative to this plan of care.
- Coordinates monthly and/or semi-annual progress of each individual relative to the established program goals and objectives, and will document all progress, lack of progress or regression in measurable terms.
- Will remain familiar with assigned individual's programming plans and protocols, current status, and family or agency interaction as it pertains to each individual.
- Maintain a uniform record keeping system that ensures consistency, confidentiality, and company standards.
- Responsible for the enforcement of Company Policies and Procedures

- Performs other related duties as requested by supervisor.
QUALIFICATIONS:
Required:
● Associates Degree in the Human Service-related field (or equivalent credits), with four to six years’ work experience in the OPWDD and serving the intellectually developmentally disabled population ,or;
● High School Diploma/GED with seven to ten years’ work experience in the OPWDD and serving the intellectually developmentally disabled population, and;
● Must be experienced in handling confidential information in the strictest manner, can multi-task and analyze problems and determine corrective measures. Self-starter, takes initiative and goal-oriented professional with the ability to work independently in an organized manner and be an effective part of a team, in a fast-paced environment with the ability to effectively collaborate with others. Evidence of ability to prioritize, manage and complete task and projects with tight deadlines and;
● Successful completion of delegated and assigned trainings determined by agency and supervisor which may include SCIP, AMAP, and CPR/First Aid certifications and familiarity with OPWDD regulations. Obtain a valid, unrestricted NYS Driver’s License in accordance with Agency Standards.
Preferred:
● Obtain previous noted requirements and;
Bachelor’s Degree in the Human Service-related field, with three to five years’ work experience in the OPWDD and serving the intellectually developmentally disabled population, and
