Key Responsibilities\n

• Independently manage a caseload of patients, evaluating their social, emotional, financial, and cultural factors that influence their hospice experience.

• Develop and implement comprehensive treatment plans aimed at overcoming psychosocial barriers to ensure a smooth transition through hospice care.

• Provide leadership within the healthcare team, addressing protective service issues, life care decisions, and advance care directives, especially for complex and high-risk cases.

• Facilitate coordination among professionals involved in patient care, ensuring that the psychosocial needs are integrated into treatment and discharge plans.

• Supervise and mentor junior staff, including MSW students and interns, providing training on effective social work interventions.
\n
• Engage with families and community agencies to ensure patients have access to necessary resources, aiming to prevent unnecessary hospital readmissions.

• Respond quickly and efficiently to psychosocial service requests, mitigating any potential delays in treatment plans and discharge outcomes.

• Participate in multidisciplinary rounds and patient care conferences to share insights on each patient’s psychosocial needs and progress.

• Adapt to emerging situations, providing crisis intervention and emotional support to patients and families during critical moments.

• Stay on-call as required to address urgent patient needs and support team assessments in real-time.

Qualifications\n

• A Bachelor’s degree in Social Work is required; a Master's degree is preferred.

Compensation and Benefits\n

The annual salary for this position ranges from USD 120,000 to 130,000, commensurate with experience and qualifications.
