Key Responsibilities\n

• Conduct initial assessments with clients to identify their goals, preferences, and available resources, forming the basis for individualized treatment strategies.

• Develop personalized rehabilitation plans tailored to each client's unique educational background, skill set, physical capabilities, and career aspirations, adapting them as necessary throughout the counseling process.

• Collaborate effectively with a diverse team of healthcare providers, including physicians, occupational therapists, and psychologists, to ensure comprehensive and coordinated care.

• Maintain meticulous case files and documentation for each client, ensuring tracking of personal details, eligibility information, insurance interfacing, and communications to enhance treatment planning.

• Monitor client progress consistently, documenting outcomes to evaluate the effectiveness of strategies and making adjustments as required to help clients achieve their rehabilitation objectives.

• Engage in ongoing communication with clients during their involvement in job training or occupational placements and manage any arising challenges promptly to support their success.

• Work alongside clients' family members to develop strategies that address additional needs and provide guidance to help them through the recovery process.

• Conduct timely assessments of client requirements and document findings comprehensively.

• Advise clients and refer them to relevant community resources as required, enhancing their support network.

Qualifications\n
