Key Responsibilities\n

• Conduct thorough psychosocial assessments utilizing interviews, observations, and an in-depth review of medical records to identify emotional, social, and environmental factors that may impact a patient’s recovery.

• Provide empathetic counseling services to patients and their families, effectively addressing issues related to adjustment, emotional resilience, and overall mental health.

• Collaborate closely with a team of rehabilitation specialists to create individualized treatment plans that honor patients’ psychosocial needs, promoting holistic recovery and independence.

• Assist patients in identifying and accessing essential community resources such as support groups, vocational rehabilitation services, and housing assistance to facilitate their post-rehabilitation transition.

• Educate patients and their families about available community resources and post-rehabilitation services, ensuring a smooth transition back to their daily lives.

• Participate actively in discharge planning, ensuring that patients have the necessary support and resources as they transition to home or other care settings.

Patient Population / Client Focus\n

Your primary focus will be on supporting adults and adolescents facing rehabilitation challenges stemming from injuries, illnesses, or disabilities. This diverse patient population requires tailored care that respects individual circumstances, emphasizing emotional support and community reintegration.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program is mandatory.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

Compensation and Benefits\n
