Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients’ emotional, social, and financial needs, assisting in the development of individualized care plans.

• Provide both individual and group counseling sessions to support patients in coping with grief, adjustment difficulties, and other emotional challenges related to their illness.

• Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care, helping to optimize patient outcomes and overall satisfaction.

• Develop discharge plans that facilitate smooth transitions from hospice care by connecting patients and families with community resources and support services.

• Contribute to crisis intervention efforts when needed, offering immediate emotional support and guidance to patients and their families.

• Maintain precise and timely documentation in alignment with HIPAA regulations and hospital policies, ensuring all records reflect the services provided.

• Participate actively in hospital committees to discuss and improve social work practices and patient care strategies.

You will primarily work with patients facing the end stages of life, providing emotional and psychological support to them and their families. Qualifications\n

• A Master's degree in Social Work from an accredited institution.
