Job Summary:

At Carease Health - NY, we are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide preventative care to residents in skilled nursing facilities, focusing on proactive measures for enhanced physical health and well-being

• Conduct on-site visits and remote work from home, with support from a dedicated scribe team for efficient documentation

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with the clinical team to ensure seamless alignment with facility processes and protocols

• Document patient progress and maintain accurate records

• Participate in training and professional development opportunities to stay up-to-date with best practices in geriatric care

• This is a part-time position of 17 hours per week

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
