Key Responsibilities\n

• Assist families in navigating community resources, connecting them with support groups, vocational services, and housing assistance as necessary.

• Educate patients and families on available community resources and post-rehabilitation services that can support long-term well-being.

• Be an integral part of discharge planning, ensuring smooth transitions for students moving back to home or other care settings.

Patient Population / Client Focus\n

You will work directly with a diverse population of students facing various challenges, including those recovering from physical injuries, neurological conditions, or psychosocial disturbances. Qualifications\n

• A Master’s degree in Social Work, obtained from an accredited program.
