Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families upon admission, annually, and as clinically necessary, to identify and address their social, emotional, and physical needs. Utilize social work theories and techniques for effective evaluation and care.

• Collaborate closely with interdisciplinary healthcare teams to design, develop, and implement individualized psychosocial treatment plans. Ensure smooth referrals to specialty and social services, maintaining detailed progress notes to support continuity of care.

• Provide patients and their families with coping strategies to manage the environmental and psychological challenges associated with illness and treatment transitions.
\n
• Independently manage a caseload of patients with varying clinical acuity, ensuring that each individual's unique needs are being met.
\n
• Participate in case finding activities to identify patients and families who may benefit from psychosocial support. Actively engage in team conferences and interdisciplinary rounds to discuss cases and develop best practices.

• Offer supportive counseling, grief counseling, crisis intervention, and therapeutic services to help patients navigate illness-related challenges and facilitate understanding of medical information and care requirements.

• Deliver psychoeducation to patients and families about coping strategies related to diagnoses, treatment options, and prognosis, particularly aiding transitions from youth to adult services.

• Engage in quality assurance initiatives aimed at improving social work services by participating in the development and review of policies, procedures, and guidelines.

• Diagnose mental health conditions as part of comprehensive treatment planning when necessary.

• Provide clinical supervision to MSWs and student interns, ensuring quality mentorship and support.
\n
• Maintain accurate and up-to-date electronic medical records in compliance with organizational policies, providing both written and oral reports as needed.

Qualifications\n

• A Master’s degree in social work or a related behavioral science from an accredited institution.

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
