Targeted Case Manager (Bilingual / Spanish)

Position Overview Hybrid LTSS Social Work / Human Services Case Manager / Targeted Case Manager Hybrid (Remote + Field) | Miami‑Dade County, FL Salary: From $34.14 per hour Schedule: Monday–Friday, 8:00 AM – 5:00 PM (no weekends, no holidays) Location: Candidates must reside in zip codes 33189 & 33190 (Hialeah). Must be willing to travel within Miami‑Dade County. Key Responsibilities Coordinate case management services for Medicaid Long‑Term Care members. Conduct comprehensive member assessments using care management tools and data review. Develop, implement, and monitor individualized care plans. Collaborate with interdisciplinary care teams to achieve optimal health outcomes. Track progress and update documentation in accordance with program requirements. Travel Requirements Up to 75% field‑based travel within Miami‑Dade County. Travel time paid at hourly wage. Mileage reimbursed at IRS rate. Reliable transportation required. Required Qualifications Case management experience. Bachelor’s degree in Social Work or related Human Services field. Must reside in Hialeah (zip codes 33189 & 33190). Bilingual Spanish/English required. Ability to work independently both in the field and remotely. Authorized to work in the U.S. Preferred Qualifications Experience in Long‑Term Care (LTC) or Medicaid programs. Strong organizational, time‑management, and multitasking skills. Excellent verbal and written communication abilities. Comfortable working in a fast‑paced, changing environment. Training 2 weeks of remote training, Monday–Friday, 8:00 AM – 5:00 PM.
