Licensed Practical Nurse (LPN)

The Licensed Practical Nurse (LPN) is responsible for assessing residents, administering medications in accordance with the 5 Rights, reporting changes in resident conditions to the charge nurse and physicians, and assisting Certified Nursing Assistants (CNAs) with their duties. The LPN also communicates updates to family members and ensures compliance with facility policies regarding resident care and safety. Responsibilities:

• Administer medications to residents following the 5 Rights protocol.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Notify the charge nurse of any changes in resident condition.
• Assist CNAs with answering call lights and other resident care responsibilities.
• Update physicians, family members, and other relevant parties about resident condition changes as needed.
• Perform additional duties as requested by supervisory staff.

• Current Wisconsin Licensed Practical Nurse (LPN) license.
• Current CPR/BLS certification.
• Exceptional accuracy and attention to detail.
• At least 1 year of related work experience preferred.

• Experience in long-term care or similar healthcare settings.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with interdisciplinary teams.

• Must comply with all facility policies regarding resident safety and abuse reporting.
• Role involves working in a long-term care environment.
• May be required to assist with urgent resident needs and respond promptly to call lights.

• Full-time position.
• Evening shift from 2:00 PM to 10:00 PM.
• Work schedule includes every other weekend.
