Hero Program Case Manager - Transitional Housing & Support

Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is hiring a Hero Program Case Manager. The Case Manager will use wrap-around philosophy to assist clients in transitional housing and coordinate individualized services under the HERO program. Responsibilities include meeting with clients, tracking the ISS/Treatment Plan of Care, collaborating with providers in home and community settings, and documenting all support activities. #J-18808-Ljbffr Catholic Charities of St. Louis
