RN Case Manager - In-Home Hospice Care

Chapters Health System in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate care with the Interdisciplinary Team, and provide clinical, palliative and supportive services to keep patients at home as long as possible. The role requires an active RN license, at least 1 year in hospice or hospital settings, experience with EMR/EHR, a valid driver's license, reliable transportation and excellent English communication. #J-18808-Ljbffr Chapters Health System
