Work Environment

This position is primarily onsite, allowing you to engage closely with patients in their home environments. Key Responsibilities

• Conduct thorough assessments of patients' health status to diagnose and manage acute and chronic conditions.
• Develop and implement individualized care plans in coordination with interdisciplinary teams, focusing on patient-centered outcomes.
• Order and interpret necessary diagnostic tests such as blood work, imaging, and other relevant assessments to inform treatment decisions.
• Prescribe medications and therapies as needed, ensuring adherence to best practices and safety protocols.
• Educate patients and their families on managing conditions, medication regimens, and preventive health measures within the home care context.
• Maintain and update accurate electronic health records to reflect changes in patient conditions and treatment plans.
• Actively engage in quality improvement projects aimed at enhancing patient satisfaction and health outcomes.
• Foster a positive working relationship with patients, families, and other healthcare providers to promote holistic care.
• Stay informed about the latest developments in nursing practices and home healthcare standards.

Patient Population / Client Focus

Your primary focus will involve working with a diverse patient population, including elderly individuals, patients managing chronic illnesses, and those recovering from surgery or hospitalization. Understanding the unique dynamics and challenges presented by home care settings will be essential to addressing patients’ health needs effectively.

Qualifications

• Current certification as a Nurse Practitioner.
• Active Texas state nursing license.
• Bachelor's degree in nursing.
• At least two years of clinical experience, preferably in a home care or similar setting.
• Exceptional communication and interpersonal skills with a strong emphasis on teamwork.
• Proficient in using electronic medical record (EMR) systems to document patient interactions and care.

Preferred Experience

• Experience in primary care or urgent care settings is highly desirable.

Compensation and Benefits
