Primary Care Nurse Practitioner (NP) - Full-Time in Assisted Living

Work Environment

The Nurse Practitioner will operate primarily onsite at our assisted living facility in Austin, Texas, within a supportive team structure. You will collaborate closely with other healthcare professionals to provide personalized and preventive care in an atmosphere that fosters open communication and teamwork. Weekly case discussions, scheduled team meetings, and access to a dedicated medical scribe will enhance your focus on patient care while administrative burdens are minimized.

Key Responsibilities

• Deliver exemplary primary care through timely office visits addressing both routine and acute health concerns.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring thorough documentation and holistic evaluations of patient health.
• Coordinate patient care with specialists, testing facilities, and other healthcare providers to ensure continuity and quality of care.
• Lead population health initiatives by ensuring adherence to preventive care guidelines, such as breast cancer screenings and chronic disease management.
• Engage in phone triage, providing informed guidance and follow-ups based on patient needs.
• Educate patients regarding their health conditions, care plans, and treatment options to foster a better understanding and active involvement in their health journey.
• Occasionally conduct home visits for patients unable to travel, ensuring their healthcare needs are addressed in a comfortable setting.
• Participate in promotional activities to enhance community awareness of our services and patient offerings.
• Work closely with the Medical Director and operational leadership to ensure high-quality patient care and organizational excellence.

Patient Population / Client Focus

Our primary focus is on serving older adults who are on Medicare within our assisted living facility. We understand that this unique population requires tailored approaches that prioritize emotional well-being and preventive care, as many patients may face chronic conditions that impact their daily lives. By establishing trust and meaningful relationships, you will support our patients in staying engaged and empowered about their health decisions.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or graduation from an accredited Nurse Practitioner program.
• Active certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through reputable organizations like AANP or ANCC.
• A valid and active Nurse Practitioner license in the state of Texas, along with a current DEA registration.
• Prior experience in a primary care setting, particularly in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
• Demonstrated ability to work collaboratively as part of a healthcare team with a passion for delivering high-quality patient care.
• Bilingual skills are a plus and will enhance patient engagement and communication.

Preferred Experience

• Experience with value-based care models and population health management is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth platforms to facilitate patient care.

Compensation and Benefits
