Work Environment

The work setting for this role is onsite at patients' homes across Austin, Texas. This unique environment allows Nurse Practitioners to assess and address the specific healthcare needs of individuals in their familiar surroundings. You will work a standard schedule from 8:00 a.m. to 5:00 p.m., totaling 40 hours each week, enabling a balanced work-life dynamic while providing critical healthcare services to our community.

Key Responsibilities

• Conduct comprehensive assessments of patients, accurately diagnosing acute and chronic medical conditions through a combination of physical examinations, health histories, and diagnostic tests.
• Develop and implement tailored treatment plans based on individual patient needs, including prescribing necessary medications and therapies.
• Educate patients and their families about disease prevention, health maintenance, and necessary lifestyle modifications to enhance overall health.
• Collaborate effectively with interdisciplinary teams, including physicians, social workers, and specialists, to coordinate care and facilitate referrals for additional services.
• Maintain precise electronic health records (EHR) and ensure that all documentation is completed in a timely manner to support continuity of care.
• Engage in quality improvement initiatives aimed at enhancing clinical practices and patient outcomes while adhering to relevant healthcare guidelines.

Qualifications

• Current certification as a Nurse Practitioner.
